Quinte Health Care’s Board of Directors recognized individuals for providing exemplary service in health care at its meeting Tuesday, however, on this occasion the Values in Action Award was given to two organizations outside of Quinte Health Care.
They were the Belleville Nurse Practitioner-Led Clinic and the Gateway Community Health Centre in Tweed.
Both partnered with QHC to provide virtual care to patients without family physicians who either needed follow-up care following a hospital stay or were dealing with minor ailments better dealt with by a family physician.
Almost 1,300 “unattached patients” were helped through the Virtual Care Program and in many cases the care kept people from being readmitted to hospital.
The Belleville Nurse Practitioner-Led Clinic is also providing follow-up care for around 100 babies born to an “unattached” patient for the first two months of their lives.
