The unit that works to make sure patients have the supports they need when they are transferred out of hospital care and back into the community, was honoured Tuesday by Quinte Health Care’s Board of Directors.
The Patient Flow Team consists of 12 people and supports all four QHC hospitals in ensuring complex-care patients are discharged in a safe, timely manner with the community supports they need.
The team looks at what the patient needs and wants, what their family wants, what resources they have, and what community resources are available to help them.
The members create discharge plans leading to a safe discharge with everyone involved knowing what to expect.
The team’s manager is Lisa Hildebrand.
